Gastrocnemius muscle architecture and achilles tendon properties influence walking distance in claudicants with peripheral arterial disease.

چکیده
INTRODUCTION The extent to which gastrocnemius muscle and Achilles tendon properties contribute to the impaired walking endurance of claudicants is not known. METHODS Ultrasound images quantified muscle architecture of the lateral and medial gastrocnemius (GL and GM) and were combined with dynamometry during plantarflexor contractions to calculate tendon stress, strain, stiffness, the Young modulus, and hysteresis. Key parameters were entered into multiple regression models to explain walking endurance. RESULTS Worse disease severity was significantly associated with longer fascicle: tendon length ratios (GL R = -0.789 and GM R = -0.828) and increased tendon hysteresis (R = -0.740). Walking endurance could be explained by GL and GM pennation angle, maximum tendon force, tendon hysteresis, and disease severity (R(2) = ∼0.6). CONCLUSIONS Peripheral arterial disease was associated with functionally important changes in muscle and tendon properties, including the usage of stored elastic energy. Interventions known to target these characteristics should be adopted as a means to improve walking endurance.

Modeling the Aging Achilles Tendon: Inter-fascicle Adhesions and Tendon Compliance Affect Plantarflexor Behavior during Walking
The Achilles tendon (AT) consists of distinct fascicle bundles arising from the gastrocnemius and soleus muscles that intertwine and twist before inserting onto the calcaneus [1]. Current musculoskeletal models simplify this architecture by representing the gastrocnemius and soleus as independent muscle-tendon actuators [2], which may be an appropriate assumption if the fascicles freely slide r...
متن کاملOptimal muscle fascicle length and tendon stiffness for maximising gastrocnemius efficiency during human walking
Muscles generate force to resist gravitational and inertial forces and/or to undertake work on the centre of mass. A trade off in muscle architecture exists in muscles that do both; the fibres should 15 be as short as possible to minimise activation cost but long enough to maintain an appropriate shortening velocity. Energetic cost is also influenced by tendon compliance which modulates the tim...

متن کاملMUSCLE BLOOD FLOW DURING EXERCISE In patients suffering from intermittent claudication,
Pathophysiological considerations predict that exercise blood flows at comparable work loads must always be reduced in claudicants in comparison to normal subjects. The present study verified that the exercise blood flow determined by 133xenon clearance measurements in the gastrocnemius muscle in claudicants and in a control group differed widely. As expected, a statistically significant correl...
متن کاملAge-related differences in Achilles tendon properties and triceps surae muscle architecture in vivo.
This study examined the concurrent age-related differences in muscle and tendon structure and properties. Achilles tendon morphology and mechanical properties and triceps surae muscle architecture were measured from 100 subjects [33 young (24 ± 2 yr) and 67 old (75 ± 3 yr)]. Motion analysis-assisted ultrasonography was used to determine tendon stiffness, Young's modulus, and hysteresis during i...
